Dear Participants and Parents/Guardians,

Our event will be photographed, recorded, and/or filmed for various purposes, including but not limited to promotional, educational, and archival use. We require you or your child consent to having your photo, video, or other digital media taken and used, please complete this opt-in form.

By completing and signing this form, you are giving consent for you or your child’s image and/or voice, video recording to be recorded and used by the event organizers.

Tamils of Northern California (TNC) came into exsistence in the early 1980's in the San Francisco Bay Area and functioned as an informal group. It was incorporated as a non-profit corporation in 1994. In 1996, the organization applied for and was confirmed by the IRS as a Federally registered non-profit organization.
